Three columns, and each one is a separately mark-bearing list. The decision column is the one to elaborate: accept as-is, accept provisionally with minor revision, or reject and re-review after major revision. Writing the grid before you write prose means a rushed answer still banks the structure, and the structure is what the marker is scanning for.
